New WLAYFC Permit - Required by all “home” teams in membership of WLAYFC who wish to participate in a friendly match against another team in membership of the SYFA

SYFA Friendly Permit - Required by all teams in membership of WLAYFC who wish to participate in a friendly match against another team not in membership of the SYFA

SYFA Tournament Permit - Required by all teams in membership of WLAYFC who wish to participate in a tournament whether at home or abroad.

Misconduct report 11-12 - Cautions report only (WLAYFC only)

Referees misconduct report - for completion by match referees.

Sports relief - All you should need to appy for a sports relief grant. (2 word documents and 2 pdf packaged as a rar file)

Excel League Autosort - For Formulating league table to auto update by simply entering the scores into the matrix.

South East Region 2011-12- For entry to the SE region, Mandatory form to be filled in.
